The dedicated and caring staff at Cascade Eyecare Center is here to serve your eye care needs in Grants Pass and the surrounding area. We offer a large optical shop, wheelchair accessible exam  rooms and the latest diagnostic equipment. Our board certified ophthalmologists provide comprehensive medical and surgical eye care, as well as routine vision exams.

Ocular surgeries are performed at our accredited outpatient surgery facility, Cascade Surgery Center, located adjacent to our office building at Cascade EyeCare Center.

A physician is always available for after-hour eye emergencies.

Physicians & Surgeons

Our team at Cascade EyeCare is made up of some of the best physicians, surgeons and eye care professionals in Southern Oregon. All of our doctors are board certified ophthalmologists. An ophthalmologist is a medical or osteopathic doctor specializing in eye health. Douglas Merritt, MD

Douglas Merritt, MD

Dr. Merritt has lived in Grants Pass since 1990. He received his medical degree from the University of Texas and completed his residency in Ophthalmology from the University of Minnesota. Dr. Merritt has volunteered his medical and surgical care to various mission organizations around the world.

 

Rod Leavitt, MD

Rod Leavitt, MD

Dr. Leavitt was born and raised in Grants Pass. He received his medical degree from Loma Linda University and completed his residency in Ophthalmology at the University of Missouri, Columbia. Dr. Leavitt offers complete medical and surgical eye care.

 

Matthew Guymon, DO

Matthew Guymon, DO

Dr. Guymon attended medical school at A.T. Still University – Kirksville College of Osteopathic Medicine in Missouri. He then completed his internship and ophthalmology residency at Michigan State University. Dr. Guymon offers complete medical and surgical eye care to patients of all ages.

 

Matthew Fullmer, DO

Dr. Fullmer received his medical degree from Midwestern University and completed his residency in Ophthalmology at Beaumont Hospital associated with Michigan State University. Dr. Fullmer offers complete medical and surgical eye care with emphasis in glaucoma procedures and surgeries.

Eyecare Center

Our Eyecare physicians provide a wide range of services to our patients, which include routine vision exams for glasses or contact lenses, comprehensive adult and pediatric eye exams, diabetic eye exams, and cataract diagnosis and surgery.

Also, Cascade Eyecare Center performs the evaluation and treatment of:

· Glaucoma
· Macular Degeneration
· Retinal and vitreous disorders
· Eyelid disorders
· Red eye and eye infections
· Dry eyes and excessive tearing
· Flashes and floaters
· Eye trauma
· Ocular involvement in neurological, rheumatologic and other systemic disorders

Eye Emergencies

The following symptoms may require immediate consultation with an ophthalmologist:
· Red or painful eye
· Flashes and floaters
· Sudden vision loss
· Chemical injury to the eye
· Foreign body or eye irritation
· Double vision
· Post-operative patients with pain, vision loss, nausea and vomiting, discharge, or increased redness

What to do in an emergency
Contact Cascade EyeCare Center immediately. If for some reason one of our physicians is not readily available to assist or direct you, go to the nearest emergency room. If you feel that you have an urgent problem that requires immediate attention, call 911 or proceed to an emergency room.

What are your hours?
Cascade EyeCare is open Monday through Friday, 8am-5pm

What should I bring to my appointment?
· A copy of your current insurance card(s) for both your vision and your medical coverage.
· Your prescription glasses if you wear them.
· If you wear contacts, please have your contacts and your back-up glasses with you. Please also bring the original box(es) the contacts came in.
· A list of medications and supplements including name, dosage and frequency.
· A list of any medication allergies.

Do you accept/bill my insurance?
We provide a courtesy billing of your claim to most insurance companies. Prior to your appointment, please contact your insurance company for verification and benefits. If your insurance requires authorization to see our physicians, please contact your primary care doctor for a written referral to our office. Should you have questions or need assistance regarding your insurance, please contact your insurance company.

Surgery Center

Cascade Surgery Center is a state-of-the-art, outpatient surgical facility, conveniently located adjacent to our clinic location in the heart of Grants Pass, Oregon. Cascade Surgery Center is Medicare-certified and accredited with the Accreditation Association for Ambulatory Health Care (AAAHC). AAAHC is a private, non-profit organization formed in 1979. They are the leader in developing standards to advance and promote patient safety, quality care, and value for ambulatory health care through peer-based accreditation processes, education, and research. A certificate of accreditation is awarded to organizations that are found to be in compliance with AAAHC’s strict standards.

Cascade Surgery Center performs a variety of eye surgeries, including:
